I think many people can sense that something is not quite right. If the relationship has been a long term one, you get to know the little things a person does, their routine.

When that routine seems to change abruptly, one can sense it. Like out of the blue needing to work on a Saturday when they have never worked a Saturday in 10 years. Or needing to go shopping but only come home with a picture frame, that never gets a picture, or worse, comes home with nothing after being gone for 4 hours and going into the bathroom for 15 minutes and you can hear the water running. Or they aren't snuggly for a day or two. Things that are out of the ordinary.

Another telltale sign, you get accused of messing around but you never did anything. That's usually a dead give away that they are feeling guilty about something they are doing behind your back.
