A true Arkansonian would never ever fill out this application. Most couldn't read it and those capable probably appreciate the little boxes for the "X" to go into but may consider it a signature block and consider each item as a binding contract.

I think that's a good idea. Most can count to at least 5 and that is about the average amount per mouth... I would stay away from fractions and decimals though
