Case Study: Sad Lisa - sensitive or delusion?

Back in the heyday of folk music, Cat Stevens produced an excellent body of work, among them, the song "Sad Lisa."

Even without the words, the music was beautiful. With the words, it was fascinating.

Is the artist describing a tale told by someone sensitive to spirits (one of the few humans said to be able to see ghosts), or the inner turmoil of one who has created an imaginary person which only they can see (ex. the federal agent in A BEAUTIFUL MIND)?
