YES If one of them not have proof of baptism Unbaptized person may not be married in traditional churches.

I don't know the official answer, of course, but I feel he/she is not under any obligation to marry anyone. Therefore, marrying a couple would most likely be at his/her discretion? If one doesn't like the idea of filling out the form, I'm sure its their prerogative to get married elsewhere...?

Priests/ministers/whatever should really give advice on how to indulge in immaculate, holy, spiritually rewarding lovemaking............. and should also be well qualified and experienced to be able to give such useful advice.

I agree...part of the protocol, to see where your beliefs are. They don't want to preform marriages that don't have the basis of what their church stands for.
