Budget is the biggest factor. Once you know how much you're likely to have available, then you can work out some kind of itinerary. Hiring a car and staying in budget motels isn't such a bad idea. If you're not freaked out about sharing, have a look at couchsurfing.
Regarding places to visit. Just chose cities that you wish to go to for a specific reason. No point in spending a day driving, just to look at nothing special, just to say you were in (insert city).

Check out Green Tortoise Tours. They do a coast to coast trip from Boston to San Francisco, or the other way round, San Fran to Boston. All off the beaten track, you'll see some amazing places, make some good friends and it's really good value for money. They do a Northern route or you can take the Southern route. I'm sure the coast to coast trips leave in September and are around two weeks in duration.

Maybe only if you've got a long distance to travel, and can't afford a flight. Otherwise, try and group all your cities together, THEN get the train to a new area once you've visited all one area.
Personally, I'd be bored sitting on a train, knowing it was going miles away from somewhere I wanted to visit, just to get to somewhere else. You lose that spur of the moment thing, as you can't just get off a train and get hold of a car that easily.

Prices here vary widely. New York and San Francisco are tremendously expensive, smaller towns nearby are often much, much cheaper. Restaurants near the attraction areas, as gift shops, etc...are always much higher than those in the neighborhoods. I find seeing a new city, state far better if I kanoodle with the regular folk. Just a thought....

I did the Baja Daze trip with them in 2009, it was great fun. A friend of mine has traveled a few times with them, he did the coast to coast this year, said it was the best trip he's ever been on, he did the southern loop, although I was advised by a friend of mine from Missouri who travels a lot, that the northern loop looks more interesting. Maybe you could do one this year, and the other next year...
Humm, 14 days... Time flies when trying to visit too many places. I would just decide on one major area of interest. The U.S. is a bit bigger in person, than it is on the map. And so much time is wasted with traveling. I avoid the major cities as much as possible and I sure wouldn't recommend big cities to a visitor: Too much traffic, bumper-to-bumper in a jam or bumper-to-bumper steady flow, either way, it is no way for one to sightsee. Ok, we have to land that airplane somewhere, so lets go with a midsize city like: Richmond, Virginia and our destination is Staunton, Virginia: Appalachian Mountains. Richmond and Staunton are about 110 miles (177km) apart. Staunton will be home base. 50 miles (80km) North of Staunton and you'll be in the middle of the Shenandoah National Park... Why I recommend this area ? I've seen many places in America, but I remember the Shenandoah valley most of all... Fall... Oh the fall time in those mountains. Every color imaginable is somewhere in the trees. Your eyes will not be able to convince your mind that what you see, is real... Beyond reality, but not a picture and it's all around you... There are several more videos on youtube about this region.

Friends have, and I saw the bus, the trip itinerary, etc...it was rad!! They create a fun atmosphere. You'd have to be a real scrooge to not like it...)

Try a glider tour in autumn, its awesome, twenty feet over the spectacular tree canopy. Dont camp on skytop mountain, god it gets cold at night.

14 days is a very short time in a big country.
Pick all the places you want to see.
Then look at the map and see where they actually are.
Then pick a few places in just one area and concentrate on them, for this trip at least.
It is better to visit a few places properly, than glance at many places.
Then, check out the transportation and accommodation in that area.
And about 50 miles (80km) southeast of Richmond, Virginia: There is Historic Colonial Williamsburg Virginia and Busch Gardens: Theme park/Amusement rides.

Some flights can last over an hour... Must use the restroom first.
