I would say, 'Darling, I have booked a luxury cruise holiday for us in 7 months time.
I am going to start working off this belly of mine on time for it, so I will feel and look good lounging by the pool.
I will need to eat healthily and go walking every evening to achieve that. But you know I am not very disciplined, so I am hoping that you will be there with me to keep me on track.

Because I care for my partner I would try to talk about that (as if it was any other thing not favorable for him) and let him realize about what is happening and I also noticed.
He would have to do the rest, I do not pamper and/ or change people, I´m a partner not a mother either a therapist.
It means he would have to take actions or not, once it´s done I´ll accept or reject whatever he decided to do ....
