I would say, just like with any generalization, that its a case-by-case thing that depends on the individuals involved.

Not often, but definitely can.
A lot depends on a couple - their compatibility and on a physical shape the "cougar" is in.
I don't think any man would be comfortable if his friends would ask him 'is it your mother-in-law?' whenever they are seen in public; hence the lady who chooses this path should be hot.
The rest - like any other couple.
