A good first date? Talking and getting along. Discovering you both have the same sense of humour for example, that you both adore the same movies, that you both worked in the same companies or towns, just at different times, that you both like Thai green curry (or whatever).
A bad one? You sit there or stand there feeling bored. You don't know what to say, you realise you have nothing in common with the other person, they don't get what you're talking about, you have no idea why they have been talking motorcycle mechanics or football for the last hour... you get a migraine and go home early.

Usually that's discovered in the first couple of emails or phone calls, but I've had dates that flopped after getting along so well via email and phone.
I marked "bowling", but for me that means any fun activity done together that allows us to talk and spend more time getting to know each other. Dinner and a movie I prefer for later into the relationship. I like my first few dates to be things like going on a trail walk, going to the zoo, playing miniature golf, or going to a museum. I like the very first one to just be meeting for coffee/tea somewhere public, though one of the best I've had recently was meeting at a Native American Pow Wow, and we enjoyed it immensely despite the rain and cold.
Kidding, I also prefer going out for a drink on a first date. I love dinner dates, but sitting through an entire dinner if you really don't hit it off with someone is awful (I'm a slow eater, hehe).
