Strange that he was calling 911 while he was drowning. I know one situation that might give you a feeling as if you are drowning and that is lungoedema. This oedema causes the lung tissue filling up with fluid and swelling so that normal lung function (= breathing) becomes impossible. One of the causes is heart failure.
