I went to vigorously pull off a clip-on tie on a best friend in elementary school. It turned out to be a regular tie and he almost chocked to death before we could cut the tie off from around his kneck.

I accidentally broke the natural gas supply line feeding my lab bench in a high school chemistry class. Someone ran out quick enough to tell an approaching priest to extiguish his pipe.
