many years ago at a previous job, my boss came back after a lunch break, and gave me an earful because i hadn't told him he was unzipped prior to him leaving for lunch.
someone at the restaurant had pointed it out to him.
i explained to him that i hadn't noticed.
didn't seem to help his mood.

I had a delivery guy come to my door one time, and I had to tell him his fly was unzipped. I think it was a UPS guy, and he'd been delivering all day long!
When my boss and I were clerking one day, a woman customer walked up to the counter, and she was unzipped. I wasn't about to say anything, so I whispered to my boss that the lady was unzipped. My boss mentioned it to her, and she was grateful. After she left, my boss thanked me for telling her.
I'm am the most absent-minded professor that ever walked the earth, and I've head a number of "incidents" such as this. I usually only discover it after I've been on a shoppig trip.

I've had a number of times when I was noticing a woman, and they suddenly turn around and see I'm admiring them. I get kind of embarrassed then.
