most of the times we hurt someone we love may not be intentional,sometimes we tend to only have tunnel vision at the moment and rather than seeing things from an objective point of view we may see it as subjective thus losing sight of the real issue then we end up hurting someone dear to us...........

and sometimes it is because we know them too well, and intentionally or otherwise know what buttons to press. in addition to the above comment.
