Hi welcome to CS. 
To succeed on a date just relax. Women can generally sense when a man is nervous or preocupied with trying to remeber the things hes supposed to do. So, just before you go on a date, take a few minutes to look in the mirror, make sure you look okay, take a few deep breaths, and remember that if it didnt work out its probably for the best.
Other than that, be yourself and ask her more about herself than you tell her about yourself.
