It's true Cin, medical insurance sometimes covers only certain percentages and costs, and there is a lot of fudging going on. This is an ongoing problem within the structure of the Health Care Insurance companies as well as the hospitals themselves. There is no doubt that there has been collusion between the two entities.
Take for example child "A" has a head injury and needs a CAT or MRI scan. The first thing a hospital asks a worrying mother is what kind of insurance they have? If you're low income, with medicare, minimal or no coverage, the scan will be performed, and most likely the gov't will be billed at "x" amt of dollars, or the hospital absorbs the cost, often times knowing the parents are unable to pay. Say they charge $500. They know exactly what they can bill the Gov't for and what amount will be paid for this scan, or the likelihood of repayment from the family.
Now child "B" needs the same exact scan, same worrying parents, but the parents have great insurance through their employer subsidized insurance, like Aetna or Blue Cross & Blue Shield. The same exact scan is performed, but the hospital then submits a bill for "Y", say $300 to the insurance company.
Why the discrepancy in the cost? Because most hospitals have a tier system for costs for the procedures they do, depending on who is paying.
There are some 1,000 insurance companies overseeing $500 Billion in revenues. The executives are being paid hundreds of millions of dollars in compensation for providing this fudging. One of their best was to eliminate coverage as soon as a condition became apparent, or impose a a max benefit payout, then refuse coverage on a pre-existing condition. The "Great American Money Pit" for sure.
Did we need some sort of Government interference? I think so. Will the Affordable Care Act solve all the problems, of course not, but at least it gives many a chance to get health care coverage without going into bankruptcy over a catastrophic illness or sudden accident. For the first time many can see Dr.'s for prevention and care, not just dropping in to visit the ER when things go south.
