Generally, I do that when I have decided the other person lacks the ability to understand why they are wrong. It's anything but polite, and in most cases, they have the ability to understand...that.

I would do it for a joke.
Example: My aunt said that my uncle would come over to her side of the bed in his sleep. But if she gave him a little bit (time) that he would go back to his side. I did a cough take and said, What? with a shocked look on my face. She stuck her tounge out at me.

Someone coughs at me, I ask them if they need a halls or a cough drop. I don't cough or ahem at anyone, I just stare at them.
