well now....
personally i wouldn't have anybody in the pic that didn't know they were gonna be on the web!..its hardly fair...
i try to put ones up of myself alone...or just my family..
that is just my 2€ worth!

Seems to me that it would depend on several things:
1. The nature of the picture; Is it fairly intimate, or just folks smiling for the camera?
2. The nature of the relationship; Are you friends? Family? Lovers? I personally wouldn't hesitate to post a pic of my best friend and I, but would NEVER post one of my lady without her permission.
3. The nature of the site where it's being posted; Seems like there'd be a big difference between posting a picture or two of friends out camping on an outdoor site and posting a pic of a significant other on a dating site.
4. If it wouldn't affect the "integrity" of the pic, (say maybe you just like the way you look in that particular pic) then the other option would be simply to crop the picture to eliminate the other person.

You could "crop" the picture, then you wouldn't have to ask anybody. Just another idea.
