To me realness doesn't come from the physical. The "real" part comes from knowing who you are as a person,not whether or not you have implants,or collagen treatments. Those are things a woman would do to feel better about themselves. A confidence builder so to speak. For some guys a real woman might be one who will go and get greasy with him under the hood of the car. Other men might consider a prim and proper woman as a real woman. The definition of "Real Woman" is undefinable. It all depends on who you ask

i guess your right it can mean alot of things to different people. i know who i am and would get greesy under a bonnet and do diy around the home. i do look after myself and dress like a woman but i am not the sort of person who would be prim and proper painting my nails and fixing my hair every day.
