Ive just counted 16 of my posts to 35 of sxc"s and i havent said one swear word or really nasty comment. Yes we know who wants the most attention dont we.
Ive never seen an official form from any tradesman but i think its a great idea. Glad you were happy with the work and i know they are not all bad.........only a small percent.
