how would you react or respond if you saw a restaurant/store or hospital employee not wash their hands before leaving the bathroom?

I'd mention it to the employee in a joking fashion as to see what his/her ( would I be in a woman's bathroom? I think notlaugh) reaction would be. That reaction would temm me if it's a habit or an omission and only upon negative feedback would I raise the issue with a manager.
Simple.

sadly I did basically just that (no humor however).

However, the place was "too busy" for the manager to address the issue right away, and summarily thanked me with a "I'll look into it sir, Thank You."

With all the diseases and bacteria in this world one would think more emphasis would be given this "simple" task of washing ones hands. I will not be going to that Applebees again.

Contact your local, (city, county or state) health department and report it. Give as many details as possible. If you have the employees name, it is even better.

IF you really wanted to raise a fuss, you could have walked out of the restroom behind (him/her) and announced in a loud voice that this employee did not wash his/her hands. Just to alert other customers. That would get the attention of management.
