Proposed to once and married him (now happily divorced). Proposed to a young friend who was agonizing about not having a woman in his life as follows: "Ryan, if you're still single when you're 50 I'll marry you." (I'll be 75!) He accepted.

I was once engaged, a little over 2 1/2 yrs ago. I found out she was being unfaithful, so the engagement ended. She was the only woman I had ever proposed to.
As for being asked, I've never had that happen.
I do hate the feeling of rejection as well, but you never know unless you asked. You got a 50/50 chance.
