I have never failed on any occasion when I have approached a woman cold....by the time I get to her table, if I haven't thought of some way of starting a conversation...I simply tell her the truth...that I have been thinking furiously of something witty to say, but for the life of me haven't been able to come up with a thing!!!....that's usually enough to break the ice and get a smile...
...the intro is one thing, but you better have something intelligent to say afterwards....
Mistake Numero Uno: ....most guys talk about themselves....the lady is probably not interested in you...she's interested to see how interested you are inher!!...show it!!...

I guess there are few things that are turn offs for me:
Someone sending me a flower saying hello gorgeous, beautiful, you are hot
Someone from another country suggesting that I visit their country within the first couple of mails I am a believer in the old school, the gentlemen is suppose to be the one doing the traveling, am I right 
People who assume to know you after a couple of chats
People who have not posted anything on their profile and try to IM me
Probably there are more things, but my mind is sort of blocked (too busy indulging in chocolate
