It certainly looks real.
My guess (and it's only a guess),
is that they originally wanted to prank their friend
and she was terrified and ran out the door to the street
and was hit (hard) by an oncoming car.
That's the way it looks to me.

Several years back when I lived in Ohio there was a prank that ended in death when a guys wife was hiding in the closet waiting to jump out and scare him. He heard some noise in the closet, went and got his gun, opened up the door and shot her.It certainly looks real.

The funniest prank I ever heard of that freaked people out was that cops caught a carload of teenage guys shooting people with red paint balls and filming it.
