In my country, Spain, we use the word heroinómano to call a heroin addict.

Commonly known as 'smackheads' round Manchester. Often see them on street corners 'rattling' (withdrawing) waiting for their dealer to turn up.

You have to want to stop. There is a medication out there though much better than methadone for dealing with the actual addiction part, but you still got you do the mental work to change your thinking.
