Sorry, hereabouts if they still share the same roof, under Common Law, that constitutes a marriage, even if they never bothered to have a marriage ceremony and in some States simply overnighting is enough of a marriage to allow alimony if one files for divorce and leaves. So yeah they may have actually divorced and separated, but once he moves back in, yeah they got married again and she isn't an Ex anything, but a current wife.

Actually a lot of folks don't anymore. Legal marriages are very much in the decline in the US. However, not all states recognize common law marriage (which is really just a hold over from hundreds of years ago when it was 'we will get married when/if a preacher comes to town' but nowadays Preachers are everywhere) so if you want legal protection in a state that doesn't do common law anymore, you need the marriage license and ceremony.

It's not that simple. There are a few states in the US that allow for common law marriage, but you can't just move in with someone and be considered a common law husband/wife. One of the conditions in Texas, for example, is that you must have publicly refered to the other person as your "husband" or your "wife" and they will quiz people before a Cl status (usually for benefits) is claimed. There are a lot more conditions. I don't know where the other poster gets his information from that you could be considered a CL couple by overnighting. I've never heard of such a thing.
