But getting back to the op. For a parent to lose a child is truly heartbreaking. When the loss is due to suicide I cant imagine the anguish for the parents, it must be unbearable.

Geno2809: Mental health care in ireland is treated like the poor relation of the HSE. Funds are constantly being cut and facilities are terrible. It is almost impossible to get mental health care for anyone u18 here.

It’s sad to say but there have been an increase in suicides over the last couple of years , mainly due to the pandemic & how it has affected peoples minds . Some of them come as a complete shock to their families, so it’s impossible to detect everyone suffering from depression & what actions people may take . Maybe at the time , the authorities felt he was stable enough to be let out into society, I don’t know . Nobody can really know what goes on in someone’s mind .

It’s not always about suffering. Some people suffer from depression even without having any trauma in their life .

It will be in his medical notes. The grieving mother can't invent "suicide watch" . It's recorded. There will be notes on his progress or lack of. Suicide watch is not something that can be "made up". It leaves a trail of recorded notes. Meetings with ward manager to make and implement the decision. Ward and psychiatrist appointed as well as date of review according to progress. Also medication prescribed etc.

Even some survivors I have spoken to don't fully know what tipped the scale.
