Many people are on suicide watch & are left to fend for themselves, as was my friend . Why was he on suicide watch?

He was safe if he took his medication, but for reasons only known to himself, he stopped taking them & the rest is history.

Sinéad O’Connor has condemned the Irish state after the death of her 17-year-old son, Shane.
The singer announced the news of her teenage son’s death on Twitter in the early hours of 8 January, writing: “My beautiful son, Nevi’im Nesta Ali Shane O’Connor, the very light of my life, decided to end his earthly struggle today and is now with God.
“May he rest in peace and may no one follow his example. My baby. I love you so much. Please be at peace.”
Shane had been reported missing two days before his death. O’Connor had previously Shane was “on suicide watch” at the Tallaght Hospital in Dublin.
In a new series of tweets on the evening of 8 January, O’Connor has fiercely criticised the Irish health service HSE, the child and family agency Tusla and the Irish state overall.
“26 hours after my son died in the so called care of the Irish State in the form of Tusla, I have yet to receive any contact from Tusla or their representatives. I was informed by Gardai of my son’s death and later I spoke with the GAL. No contact from Tusla is unacceptable,” she wrote in her first tweet.
O’Connor later posted: “I have now formally identified the remains of my son, Shane. May God forgive the Irish State for I never will.”
The “Nothing Compares 2 U” singer added: “Now tusla want to discuss with me ‘a media release’ no doubt wishing to have me join in their efforts to make this death of my child seem like it wasn’t at the hands of the Irish State.”

.I've no idea what Tusla are like, or what they're like to deal with, but what I do know is that suicide amongst our young men (less so girls) is a huge issue in Ireland and had needed an urgent national discussion for decades now.
I knew /know of far too many young men who took their own lives or were caught just in the nick of time..My youngest son lost three classmates over the course of two weeks, it really shook him and his classmates. And almost every week before that and after, we heard of another young man who took his own life...it really scared me as a mother of young teenage sons so I regularly talked to them about it and made sure they knew I could handle anything as long as they were alive...that no matter how bad it was, to come and talk to me, because as long as they're breathing it can be fixed.....
As a mother of sons my heart goes out to Sinead. The poor woman had a truly dreadful childhood and it left deep deep wounds on her soul...but that does NOT mean her parenting had anything to do with her son's suicide.....although god love her it probably didn't help matters..How does she get through this....how does any mother/father ever get over the loss of their child...I truly hope the country is looking after her because God knows she's always been a fragile soul has our Sinead.....my thoughts are with her ..

Some of them come as a complete shock to their families, so it’s impossible to detect everyone suffering from depression & what actions people may take .
Maybe at the time , the authorities felt he was stable enough to be let out into society, I don’t know .
